Peter Rosier

Senior Lecturer, Functional Urology and Neurourology, Department of Urology, University Medical
Peter Rosier is currently the Medical Staff, Senior Lecturer at the
Department of Urology, University Medical Centre, Utrecht
The Netherlands.
In the days I started in the urology department as a young resident, there were some clinkcal phase 2 studies coming up within a few months for the treatment of BPH with 5AR, with alpha blocker, and for the treatment of an overactive detrusor with neuromodulation. The professor and chair of my department at the time thought that the indication and the effect of these treatments should be determined with objective criteria. Urodynamic testing was still moderately developed clinically for people without dysfunction due to neurological abnormalities, but applicable. I started, measuring bladder outflow obstruction, quantifying detrusor contraction, and quantifying detrusor overactivity. After a few years, I published a dissertation that resolved some of the controversies that existed at the time, in particular regarding the different parameters to quantify bladder outflow obstruction. But also through this work I saw how bladder contraction force and bladder outflow obstruction were related. I continued working in this area of research and continued to understand that it is very fruitful to listen to the patient and have a chat about her or his bother about the symptoms, but also understood that listening to the patient never can lead to a diagnosis of the pathophysiology that causes the symptoms, especially not in patients where initial treatment options fail.
